MINISTRY DESCRIPTION: Abba’s Heart Encounters is a biblically based Heart Healing Ministry providing prayer, inner healing, and spiritual ministry. This ministry is considered pastoral and spiritual care, not professional, clinical, or licensed counseling. Participation in all ministry sessions, events, retreats, or activities is voluntary and conducted only as you choose to engage. If you are experiencing serious physical illness, mental health conditions, emotional disorders, trauma, or addiction, you are strongly encouraged not to discontinue or disregard professional medical or mental health care, and to seek such care alongside any prayer or ministry you receive. CONFIDENTIALITY & MANDATORY REPORTING Abba’s Heart Encounters is committed to honoring your privacy and keeping personal information confidential. However, Texas law requires mandatory reporting of any current physical or sexual abuse involving a minor. MEDICAL DISCLAIMER We are not licensed medical or mental health professionals. This ministry is not intended to diagnose, treat, cure, or prevent any medical or mental health condition, nor is it a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a licensed physician or qualified healthcare provider regarding any medical or mental health condition. Never disregard or delay seeking professional care because of information, prayer, or ministry received through this ministry. LIABILITY RELEASE NOTICE (INCLUDING RETREATS & EVENTS) You must sign and date this waiver for your registration to be complete and your spot to be reserved. I acknowledge that by signing this document, I am giving up certain legal rights, including the right to sue Abba’s Heart Encounters, and, where applicable, its ministry team, retreat hosts, property owners, representatives, volunteers, facilitators, speakers, and affiliates, for any injuries, damages, losses, or claims that may occur during or as a result of my participation in ministry sessions, events, or retreats. ASSUMPTION OF RISK I understand that participation in heart healing ministry, prayer sessions, deliverance ministry, retreats, travel, lodging, group activities, worship, ministry exercises, and related events carries inherent risks. These risks may include, but are not limited to: • Emotional or psychological distress • Spiritual discomfort • Physical injury • Illness • Property loss or damage • Accidents during activities, transportation, lodging, or free time. I understand that these risks may arise from my own actions, the actions of others, environmental conditions, or unforeseen circumstances. I knowingly and voluntarily assume all such risks, including risks that may not be known or anticipated at this time, and I choose to participate regardless. RELEASE & HOLD HARMLESS I hereby release, waive, discharge, and covenant not to sue Abba’s Heart Encounters from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, damage, emotional distress, illness, or death that may occur to me or my property during or after my participation, whether caused by negligence or otherwise. I further agree to indemnify and hold harmless Abba’s Heart Encounters from any loss, liability, damage, or costs, including court costs and attorney’s fees, arising from my participation. REPRESENTATION & ELIGIBILITY I represent that: • I am 18 years of age or older • I am emotionally, physically, and medically able to participate • I am participating voluntarily • I have read and fully understand this waiver and release. I understand that this waiver applies to me and anyone legally representing me, including my spouse, family members, heirs, assigns, and personal representatives. SEVERABILITY & GOVERNING LAW If any portion of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect. This waiver is intended to be as broad and enforceable as permitted by law and shall be governed by and construed in accordance with the laws of the State of Texas. FINAL ACKNOWLEDGMENT By signing below, I acknowledge that I have read this document in its entirety, understand its contents, and sign it voluntarily as my own free act and deed. No oral representations or promises have been made beyond what is contained in this written agreement.

CONSENT TO RECORD: We love you and want to capture memories from our gatherings! By signing this form, I grant permission to Abba’s Heart Encounters, including its ministry team members, facilitators, prayer leaders, volunteers, retreat hosts, and representatives, to photograph, video record, and/or audio record me during ministry-related activities, including but not limited to retreats, conferences, training sessions, mission trips, worship gatherings, and other scheduled events. USE OF MEDIA: I grant Abba’s Heart Encounters the right to use these photographs, video recordings, and audio recordings, in whole or in part, in any medium now known or later developed, including but not limited to: • Websites and online platforms • Social media • Print materials • Promotional videos • Presentations • Newsletters, brochures, and marketing materials • Retreat photo galleries or portfolios. These materials may be used for promotion, education, documentation, or marketing purposes related to Abba’s Heart Encounters. EDITING & USE DISCRETION: I understand that Abba’s Heart Encounters may edit, crop, enhance, or modify images or recordings for quality, clarity, length, or formatting, provided that the overall intent is not misrepresented. I further understand that Abba’s Heart Encounters is under no obligation to use any image or recording taken of me. IMPORTANT PRIVACY ASSURANCE: Abba’s Heart Encounters will NOT share or publish video or audio recordings of personal, one-on-one, or private ministry sessions on its website, social media platforms, or promotional materials. I understand and agree that group photos or group video footage, including images taken during retreat settings or group ministry environments (without focus on personal ministry content), may be used for the purposes stated above. ASSUMPTION OF PUBLIC EXPOSURE: I understand that once images or recordings are published online, particularly on websites or social media platforms, Abba’s Heart Encounters cannot control third-party sharing, reposting, or redistribution. INTERNATIONAL & MISSION USE: I understand that images or recordings may be used in international contexts, including mission-related materials or global ministry platforms, without geographic limitation. COMPENSATION & RELEASE: I understand that I will not receive any compensation for the use of my image, voice, or likeness. I hereby release, waive, and hold harmless Abba’s Heart Encounters, including its staff, volunteers, prayer leaders, facilitators, retreat hosts, and representatives, from any claims, demands, or legal actions arising from the authorized use of my image, voice, or likeness as described in this agreement. REVOCATION & CONSENT: I understand that I may revoke this permission at any time by submitting a written request to Abba’s Heart Encounters. I acknowledge that revocation applies only to future use and does not affect media already published, distributed, or in circulation prior to receipt of my written request. MINORS: This Media Release Agreement applies only to participants eighteen (18) years of age or older. A separate parent or legal guardian consent form is required for minors. GOVERNING LAW: This agreement shall be governed by and construed in accordance with the laws of the State of Texas. ACKNOWLEDGEMENT & AGREEMENT: By signing below, I acknowledge that: • I have read and understand this Media Release Agreement • I am 18 years of age or older • I am signing voluntarily and of my own free will • No oral promises or representations have been made beyond this written agreement.
